Name a custom field when creating it inline from the question builder

When adding an application question and clicking Create & map custom field, you can now type a name for the new field right there — no need to open field settings afterwards to rename it.

• Inline name input — a “Custom field name” text box appears above the create button; leave it blank and the field name still defaults to the question prompt.
• No extra steps — the identifier is still auto-generated from whatever name you provide, just as before.

No admin action required.
