More reliable verification code entry

The verification code screen — used when prospects confirm their phone or email, and when advocates and admins log in — is back to a single text field instead of the separate digit boxes.
The boxed input looked nice, but it broke the built-in code autofill on several platforms: iPhone and Mac would fill only the first digit (or show no autofill prompt at all), and Android’s one-tap fill was unreliable. A single field is what the operating systems actually fill.
