Track waiver versions and per-version signing counts

When you correct or update a published waiver, Stoked now tracks which version each person signed — without changing what anyone already agreed to.
- Version bumps on edit-after-signing — once at least one person has signed the current waiver, your next change to the title or agreement advances the version number. Fixing a typo before anyone signs still edits in place — no new version.
- Per-version signing counts — each waiver template page now has a Versions section showing how many people signed each version (“6 signed v1, 15 signed v2”), and the Submissions table shows the version each person signed. To see exactly what a past version said, open one of its signed submissions.
- Every signature is fully self-contained — each submission records the agreement, the questions asked, and the answers given at the moment of signing, so a signed waiver always reflects what that person actually agreed to (and is the source of truth for what each version looked like).
- Signing links always use the current version — prospects continue to sign the latest published wording.
No action required. Your existing waivers stay as version 1, and the version number advances the first time you edit after a signature.
